It's a sequel to Tsukihime, which is also loosely connected to Kara no Kyoukai. I don't think they share the same universe as Fate though but I may be wrong
It's a prequel, actually. This takes place in the 1980s, where Tsukihime takes place in 1999-2000. Aoko is also a high schooler here, where she's in her mid 20s-early 30s when she meets Shiki in the prologue of Tsukihime. This is also the first story written for the "Nasu-verse", then coming Kara no Kyoukai, then Tsukihime, then Fate.
But yes, they all share a universe. Fate offhandedly mentions some of the characters of these games, and some (like Touko, Aoko and Arcueid) appear in Fate/Extra.
