As for your second question, for me personally, its both. I absolutely adore a "forbidden love" cliche in otome games, but ofcourse plot and good writing is essential
I like playing otome/GxB games and even BxB if the story intrigues me. I do find the plots about incest weird so I kind of avoid them when I'm not daring enough, but I have played some games with darker themes that caught my interest. I enjoy childhood friends or non related characters who grew up as brother and sister content. Also a fan of more mature stories as well cause most VN's targeted for the female audience are too innocent or silly.
I usually play this kind of game when there is NO "brother, cousin" issue (something that I find extremely unpleasant to have this kind of relationship in the family).
If it is nothing related to that, the story is usually good.
About "my lovely little sister" gave me a real trigger. This is probably a game I won't play.

A shame indeed. Some VN's have awesome stories, but the lewd stuff can sometimes clash with the aesthetics. Examples which come to mind are Yuno and Fate/Stay.
